PeKa Sihat (formerly PeKa B40) 2026: eligibility, free health screening and benefits of up to RM20,000

A black and silver stethoscope on a light blue background, with the title PeKa Sihat (formerly PeKa B40) 2026 on a dark blue panel

PeKa B40 is now called PeKa Sihat. The name has changed but the scheme is the same: a Ministry of Health (MOH) initiative run by ProtectHealth to help the B40 group detect and treat non-communicable diseases such as diabetes, high blood pressure and high cholesterol.

Note: as of November 2026, the MOH and MyGOV pages still use the old names PeKa B40 and Bantuan Prihatin Rakyat (BPR, now STR). This guide follows ProtectHealth's current names: PeKa Sihat and STR.

If you (or your spouse) are a Sumbangan Tunai Rahmah (STR) recipient aged 40 or over, you qualify automatically: no special registration and no charges. This guide covers the four benefits, how to check eligibility, what happens at the clinic and common questions. Every figure comes from ProtectHealth's official pages and statements and the Budget 2027 speech (sources below).

What is PeKa Sihat, and why is it no longer "PeKa B40"?

PeKa stands for Skim Peduli Kesihatan untuk Kumpulan B40 (Health Care Scheme for the B40 group). MOH introduced it in April 2019 and it is run by ProtectHealth Corporation, an MOH-owned company. In its 9 October 2026 media statement ProtectHealth calls the scheme 'PeKa Sihat (previous known as PeKa B40)', and the old PeKa B40 page now redirects to the PeKa Sihat page.

The scheme focuses on non-communicable diseases (NCDs). According to ProtectHealth, of the 328,302 people screened under PeKa Sihat in 2025, 44.5% had at least one previously undetected health concern. By 31 August 2026, more than 2 million Malaysians had been screened since 2019.

Budget 2027The Budget 2027 speech (9 October 2026, paragraph 88) proposes RM80 million to continue the scheme in 2027 (called 'PeKa B40' in the speech). This is a proposed allocation that still has to be passed, but it shows the scheme continues next year.

Who qualifies for PeKa Sihat in 2026

According to the ProtectHealth FAQ, there are only two conditions:

There is no special registration. If you receive STR (see the STR 2027 guide), you are already in the system. Your spouse also qualifies if registered in the STR application. All benefits are free.

Under 40 or not an STR recipient? You do not qualify for PeKa Sihat for now. ProtectHealth says the scheme began as a pilot carried out in stages and may be expanded depending on its effectiveness and government funding. Meanwhile, check whether you can apply for STR, since STR is the gateway to PeKa Sihat and mySalam.

The 4 PeKa Sihat benefits in one table

Benefit 1 (screening) is the gateway: you must complete the screening before you can get benefits 2, 3 and 4, and applications for benefits 2 to 4 are made through MOH hospitals only.

BenefitValueHow you get it
1. Health screeningFree (once)Bring your MyKad to a PeKa Sihat panel clinic
2. Medical device aidUp to RM20,000 per personMOH hospital doctor refers you to a Medical Social Work Officer
3. Cancer treatment completion incentiveRM1,000 (RM300 + RM700)Applied for by your treating doctor at an MOH hospital
4. Transport incentiveUp to RM500 (Peninsular) / RM1,000 (Sabah, Sarawak, Labuan)Applied for with benefit 2 or 3, based on distance from home to hospital

Benefit 1: free health screening, step by step

  1. 1. Check eligibility on the PeKa Sihat eligibility portal with your MyKad number.

  2. 2. Find a panel clinic: participating private clinics display a PeKa Sihat logo sticker on the front door. The clinic list is also on the PeKa Sihat page ('Find PeKa Sihat Clinics'). According to ProtectHealth's Budget 2027 media statement (9 October 2026), its network includes 5,909 private general practitioners.

  3. 3. First visit: bring your MyKad. No early registration and no fasting needed. The doctor takes your medical history, does a physical examination and a mental health screening, and takes blood and urine samples.

  4. 4. Second visit: the doctor explains your lab results. If needed, you are referred to a Klinik Kesihatan or MOH hospital with a PeKa Sihat referral letter. The clinic will contact you by phone, SMS or WhatsApp if no date was set.

Lab tests listed by ProtectHealth: full blood count, kidney profile (eGFR), HbA1c (blood sugar control), lipid profile (cholesterol) and urine tests. A clinical breast examination for women and a prostate examination for at-risk men are also included.

Only oncePeKa Sihat screening can only be done once per recipient, as long as funds last. If you go to another clinic for a second screening, it will not proceed. Choose a clinic you can easily return to for the second visit.

Benefit 2: medical device aid of up to RM20,000

After screening, if a specialist at an MOH hospital confirms you need a medical device, the doctor refers you to a Medical Social Work Officer to apply. The maximum is RM20,000 per person, subject to the price range in each category. Any balance can be used for another device later (for example intraocular lenses first, a pacemaker later), but it cannot be transferred to family members.

ProtectHealth's official PeKa Sihat page lists 10 medical device categories:

Benefits 3 and 4: the RM1,000 cancer incentive and transport

The cancer treatment completion incentive is worth RM1,000, to encourage patients to finish their treatment at MOH hospitals. It is paid in two stages straight into the same account as your STR payments:

StageAmountWhen
FirstRM300When the application from the MOH hospital is received
SecondRM700After you complete the doctor's treatment plan with no missed sessions
TotalRM1,000

Your treating doctor applies on your behalf. Traditional or alternative treatments are not covered, and the incentive cannot be claimed by relatives.

The transport incentive helps with the return trip to an MOH hospital for cancer treatment (benefit 3) or treatment involving a medical device (benefit 2), on your appointment dates. The amount depends on the distance from home to hospital, up to RM500 in Peninsular Malaysia and RM1,000 in Sabah, Sarawak and Labuan. It is paid into your account before your next follow-up. If you do not attend treatment, it is not paid.
